This camp must comply with regulations of the Massachusetts Department of Public Health and be licensed by the local Board of Health.

*In order to complete the registration for this camp, please submit copies of your child's most recent/current (past 24 months) Physical and Immunization Records* 

Records must be submitted at least 1 week prior to the start of camp. Without these records we are unable to have your child participate – It is a Massachusetts State requirement and is enforced by each town across the commonwealth.

When submitting your child’s records be sure to highlight their name, which camp they are participating in and the camp week itself.  

You can either mail in their records or scan and send via email.
